Designing materials for group appointments for Type 1 Diabetes
Project Background
Adolescents and young adults often have a difficult time as they take on more responsibility for their diabetes. It can also be very challenging for clinicians to help these patients navigate the transition. A group of diabetes clinicians developed a group appointment model for this patient population and asked us to help them with new branding and content to make it easy for other clinics to use.
Our Work
We created the Team Clinic Toolkit which includes a clinic manual, patient education materials, workshop facilitation guides, and more.
